2. Perfecting Smiles Payment Plan
ONLY for patients without Dental Insurance coverage
The Perfecting Smiles Payment Plan (PS Payment Plan) is only offered by Dr. Bolen's office located at 755 Commerce Drive, Ste. 403, Decatur, GA.
To participate in the PS Payment Plan you must pay 60% of your treatment balance prior to treatment. We will bill the remaining 40% to you in 2-4 installments.
The Perfecting Smiles Reduced Fee Plan(PS Reduced Fee Plan) is only offered by Dr. Bolen's office located at 755 Commerce Drive, Ste. 403 ,Decatur, GA.
To participate in the PS Reduced Fee Plan: a one time premium of $300. An additional $200 for each adult added to your plan. An additional $100 for each child added to your plan.
The PS Reduced Fee Plan is 15% less than Office Customary Fees. And includes your first Cleaning, Exam and X-rays covered at 100%.
The PS Reduced Fee Plan does not apply to all cosmetic treatments for example bleaching, and Invisalign.
